Vardenafil and Doxazosin: Understanding the Interaction
Published: 30 October 2026
Vardenafil and doxazosin can be taken together, but only under careful medical supervision because both medicines lower blood pressure and the combination increases the risk of dizziness or fainting. Vardenafil is a PDE5 inhibitor used to treat erectile dysfunction, while doxazosin is an alpha-blocker prescribed for high blood pressure or prostate enlargement. Your prescriber will check that you are stable on doxazosin and will usually start you on the lowest dose of vardenafil to minimise the risk of a sudden drop in blood pressure.
Stand up slowly
When taking vardenafil and doxazosin together, always stand up slowly from sitting or lying down to reduce the risk of dizziness or fainting. Sit or lie down immediately if you feel lightheaded.
What is vardenafil and how does it work?
Vardenafil (generic Levitra) is a prescription medicine used to treat erectile dysfunction in adult men [5]. It belongs to a group of medicines called PDE5 inhibitors, which work by relaxing blood vessels in the penis, allowing increased blood flow when you are sexually aroused. Vardenafil does not cause an erection on its own; sexual stimulation is still needed for it to work.
Vardenafil is available in 5 mg, 10 mg and 20 mg tablets. The usual starting dose is 10 mg, taken about 25 to 60 minutes before sexual activity [5]. You should not take vardenafil more than once a day. The medicine can help you get and keep an erection suitable for sexual activity, but it does not cure erectile dysfunction or increase sexual desire.
Common side effects include headache, flushing, indigestion and a blocked or runny nose [5]. Vardenafil is not suitable for everyone, particularly men taking nitrates for chest pain, those with severe heart or liver problems, or anyone who has had a recent stroke or heart attack. A prescriber will review your medical history and current medicines before deciding if vardenafil is safe for you.
What is doxazosin and why is it prescribed?
Doxazosin is an alpha-blocker medicine prescribed for two main conditions: high blood pressure (hypertension) and symptoms of an enlarged prostate gland (benign prostatic hyperplasia) [12]. It works by relaxing smooth muscle in blood vessel walls and in the prostate and bladder neck, which lowers blood pressure and improves urine flow.
Doxazosin is usually started at a low dose, such as 1 mg once daily, and increased gradually over several weeks to reduce the risk of dizziness or fainting when you first stand up (postural hypotension). The dose may be increased to 2 mg, 4 mg or 8 mg depending on your response and the condition being treated. It is important to take doxazosin regularly, usually at bedtime, to keep your blood pressure controlled or your urinary symptoms under control.
Common side effects include dizziness, tiredness, headache and swelling of the ankles. Because doxazosin lowers blood pressure, you may feel lightheaded when you stand up, especially when you first start treatment or after a dose increase. Your doctor will monitor your blood pressure and adjust the dose as needed to balance effectiveness and side effects.
Why men on doxazosin may need ED treatment
Many men taking doxazosin for prostate symptoms or high blood pressure also experience erectile dysfunction. ED becomes more common with age and is often linked to the same underlying health conditions, such as cardiovascular disease and diabetes, that lead to high blood pressure or prostate enlargement [1]. Doxazosin itself does not usually cause ED, but the conditions it treats and other medicines you may be taking can contribute to erection problems. Treating ED with a medicine like vardenafil can significantly improve quality of life, but the combination with doxazosin requires careful prescribing to avoid unwanted blood pressure drops.
When to get urgent help
- Erection lasting more than 4 hours (priapism) – call 999 or go to A&E immediately
- Sudden loss of vision or hearing – seek urgent medical help
- Chest pain during or after sex – do not take nitrates; call 999 if pain persists
- Severe dizziness or fainting after taking both medicines – sit or lie down and contact your GP or NHS 111
- Allergic reaction (swelling of face, lips, tongue or throat, difficulty breathing) – call 999
Call 999 or go to A&E in an emergency, or contact NHS 111 for urgent advice.
How do vardenafil and doxazosin interact?
Vardenafil and doxazosin both lower blood pressure, so taking them together can increase the risk of symptomatic hypotension—a sudden drop in blood pressure that causes dizziness, lightheadedness or fainting [5]. This interaction happens because vardenafil relaxes blood vessels in the penis and throughout the body, while doxazosin relaxes blood vessels and smooth muscle in the prostate and bladder. When the blood-pressure-lowering effects combine, you may feel dizzy or unsteady, particularly when standing up quickly.
The interaction is well recognised and manageable, but it requires caution. Clinical studies have shown that starting vardenafil at a low dose (5 mg) in men who are already stable on doxazosin minimises the risk of significant blood pressure changes [5]. Your prescriber will check that your doxazosin dose has been stable for at least a few weeks before adding vardenafil, and they will usually recommend the lowest effective dose of vardenafil to start with.
It is important to tell your prescriber about all medicines you take, including over-the-counter products and herbal supplements. If you experience severe dizziness, fainting or feel unwell after taking vardenafil and doxazosin together, sit or lie down immediately and contact your GP or NHS 111 for advice. Do not drive or operate machinery if you feel dizzy.
The mechanism behind the interaction
Both vardenafil and doxazosin cause vasodilation—widening of blood vessels—which reduces the resistance to blood flow and lowers blood pressure. Doxazosin blocks alpha-1 receptors on smooth muscle cells in blood vessel walls, the prostate and the bladder neck, causing relaxation. Vardenafil inhibits the enzyme phosphodiesterase type 5 (PDE5), which increases levels of a chemical called cyclic GMP that also relaxes smooth muscle and dilates blood vessels, particularly in the penis. When both mechanisms are active at the same time, the combined vasodilation can lead to a more pronounced drop in blood pressure than either medicine alone.
| Medicine | Interaction with vardenafil | What it means for you |
|---|---|---|
| Doxazosin (alpha-blocker) | Both lower blood pressure; increased risk of dizziness or fainting | Start vardenafil at the lowest dose (5 mg) only when stable on doxazosin; stand up slowly; prescriber must approve |
| Tamsulosin (alpha-blocker) | Both lower blood pressure; similar interaction to doxazosin | Start vardenafil at the lowest dose; prescriber will check you are stable on tamsulosin first |
| Alfuzosin (alpha-blocker) | Both lower blood pressure; increased risk of symptomatic hypotension | Caution required; lowest vardenafil dose; prescriber must assess stability and blood pressure |
| Terazosin (alpha-blocker) | Both lower blood pressure; additive effect increases dizziness risk | Start vardenafil at 5 mg only when stable on terazosin; monitor for dizziness when standing |
| Other blood pressure medicines (e.g. amlodipine, ramipril) | Additive blood-pressure-lowering effect; usually manageable | Declare all blood pressure medicines during consultation; prescriber will check it is safe to combine |
What your prescriber will check before prescribing vardenafil and doxazosin together
Before prescribing vardenafil to someone taking doxazosin, your prescriber will carry out a thorough assessment to ensure the combination is safe for you [2]. They will ask about your current doxazosin dose, how long you have been taking it, and whether your dose has been stable. It is important that you are well established on doxazosin—usually for at least a few weeks—before starting a PDE5 inhibitor, as this reduces the risk of a sudden blood pressure drop.
Your prescriber will also review your blood pressure readings, any history of dizziness or fainting, and other medicines you are taking. They will check for contraindications to vardenafil, such as use of nitrates (GTN spray, isosorbide mononitrate or dinitrate), nicorandil, riociguat or recreational drugs called poppers (amyl or alkyl nitrites), all of which must never be taken with vardenafil because the combination can cause a dangerous fall in blood pressure. They will also ask about recent heart problems, stroke, severe liver or kidney disease, and any history of sudden vision or hearing loss.
If the combination is considered safe, your prescriber will usually start you on the lowest dose of vardenafil (5 mg) and advise you to take it at a different time of day from your doxazosin if possible, to reduce the chance of both medicines having their maximum blood-pressure-lowering effect at the same time. You will be given clear instructions on warning signs to watch for and when to seek help.

Practical advice for taking vardenafil and doxazosin safely
If you have been prescribed vardenafil while taking doxazosin, follow your prescriber's instructions carefully. Start with the lowest recommended dose of vardenafil (usually 5 mg) and take it about 25 to 60 minutes before sexual activity [5]. Do not take vardenafil more than once in 24 hours. Take your doxazosin at the same time each day as prescribed, usually at bedtime, and do not stop or change the dose without consulting your doctor.
Be aware of the signs of low blood pressure: dizziness, lightheadedness, feeling faint, blurred vision or weakness. These symptoms are most likely when you stand up quickly, so get up slowly from sitting or lying down, especially after taking vardenafil. If you feel dizzy, sit or lie down until the feeling passes. Avoid drinking large amounts of alcohol, as this can make low blood pressure worse and reduce the effectiveness of vardenafil.
Do not take vardenafil if you have taken a nitrate medicine (such as GTN spray or isosorbide mononitrate) for chest pain, or if you use poppers, as the combination can be life-threatening. If you develop chest pain during or after sex, do not take a nitrate; sit down, rest, and call 999 if the pain does not go away. Keep all your follow-up appointments so your prescriber can monitor your response and adjust your treatment if needed.
When to contact your doctor or pharmacist
Contact your GP or pharmacist if you experience persistent dizziness, fainting, or if you feel unwell after taking vardenafil and doxazosin together. If you have an erection lasting more than four hours (priapism), sudden loss of vision or hearing, or chest pain during sex, seek urgent medical help by calling 999 or going to A&E immediately. Do not wait. If you are prescribed a new medicine or your doxazosin dose is changed, check with your prescriber or pharmacist whether it is still safe to take vardenafil. Never share your medicines with others, even if they have similar symptoms.
Other erectile dysfunction treatments and doxazosin
If vardenafil is not suitable or well tolerated, other PDE5 inhibitors may be considered, but all carry a similar interaction risk with doxazosin [3][4][5][6]. Sildenafil (Viagra) and tadalafil (Cialis) are also used with caution in men taking alpha-blockers; the prescriber will start with a low dose and monitor for blood pressure changes. Avanafil (Spedra) is another PDE5 inhibitor with a faster onset of action, but the same precautions apply.
Your prescriber may consider switching you to a different alpha-blocker, such as tamsulosin, which is more selective for the prostate and may have a lower risk of blood pressure interaction, although caution is still needed. Alternatively, non-oral treatments for ED, such as vacuum erection devices, alprostadil cream (Vitaros) or alprostadil injections (Caverject), do not interact with doxazosin in the same way and may be an option if oral medicines are not suitable [7].
Lifestyle changes can also help improve erectile function and reduce the need for higher doses of ED medicines. Losing weight if you are overweight, exercising regularly, stopping smoking, reducing alcohol intake and managing stress can all improve blood flow and erectile function [1]. Your GP or pharmacist can provide advice and support to help you make these changes, and treating underlying conditions such as diabetes or high cholesterol can also make a significant difference.
