Published: 16 October 2026
Many men wonder whether sildenafil and amiodarone can be taken together safely. The good news is that sildenafil (Viagra) is not absolutely contraindicated with amiodarone, but the combination does require careful prescriber assessment because both medicines can affect your heart rhythm and blood pressure. If you take amiodarone for an irregular heartbeat, your doctor or pharmacist will need to review your full medical history and current heart health before prescribing any erectile dysfunction treatment.
At a glance
What is amiodarone and why does it matter for ED treatment?
Amiodarone is a prescription medicine used to treat serious irregular heart rhythms (arrhythmias), including atrial fibrillation and ventricular tachycardia. It works by slowing down electrical signals in the heart to help restore a normal, steady rhythm. Because amiodarone affects the heart's electrical activity and can interact with many other medicines, it's important to tell your prescriber if you're taking it when seeking treatment for erectile dysfunction.
Amiodarone can cause a range of side effects and has a very long half-life, meaning it stays in your body for weeks or even months after you stop taking it. It can also affect the levels of other medicines in your bloodstream by interfering with liver enzymes that break down drugs. This is why any new medicine, including sildenafil or other ED treatments, must be carefully considered by a healthcare professional who knows your full medication list and heart condition.
What is sildenafil and how does it work?
Sildenafil is the active ingredient in Viagra and is a type of medicine called a phosphodiesterase type 5 (PDE5) inhibitor. It's licensed in the UK to treat erectile dysfunction by helping to relax the blood vessels in the penis, allowing more blood to flow in when you're sexually aroused. Sildenafil does not cause an erection on its own; sexual stimulation is still needed for it to work.
Sildenafil is usually taken about one hour before sexual activity, and its effects can last for around four to five hours. It's available in 25mg, 50mg and 100mg tablets, and the usual starting dose is 50mg. Because sildenafil affects blood vessels throughout the body, it can lower blood pressure slightly, which is why it's important to check for interactions with other medicines that also affect your heart or blood pressure, including amiodarone.
How sildenafil affects blood pressure and heart rhythm
Sildenafil causes a small, temporary drop in blood pressure in most men, which is usually well tolerated. However, in people taking certain heart medicines or those with underlying heart conditions, this effect needs careful consideration. Sildenafil can also very rarely affect the heart's electrical activity (the QT interval on an ECG), although this is not common at standard doses. Your prescriber will assess whether your heart is healthy enough for sexual activity and whether sildenafil is suitable given your other medicines and conditions.
Can you take sildenafil and amiodarone together?
Sildenafil and amiodarone can be prescribed together, but only after a thorough medical assessment by a qualified prescriber. Unlike nitrates, nicorandil or riociguat, which are absolutely contraindicated with sildenafil, amiodarone is not an automatic 'no'. However, there are two important considerations: first, both medicines can affect the heart's electrical activity (specifically, they may prolong the QT interval on an ECG), which in rare cases can lead to a serious irregular heartbeat. Second, amiodarone is a moderate inhibitor of the liver enzyme CYP3A4, which means it can slightly increase the levels of sildenafil in your bloodstream.
Because of these potential interactions, your prescriber will need to know about your amiodarone dose, how long you've been taking it, your underlying heart condition, and any other medicines you take. They may start you on a lower dose of sildenafil (such as 25mg) and monitor you more closely. It's essential that you're honest and complete when filling in your online consultation, as this allows the prescriber to make a safe, individualised decision about whether sildenafil or an alternative ED treatment is right for you.
What your prescriber will check
During your consultation, the prescriber will ask about your heart condition, why you take amiodarone, any symptoms such as dizziness or fainting, and whether you've had any recent ECG or heart monitoring. They'll also check your blood pressure, other medicines (especially other drugs that affect the QT interval), and whether you've had any side effects from amiodarone. This thorough review helps them decide if sildenafil is safe for you, whether a dose adjustment is needed, or whether a different ED treatment might be more suitable.
| Medicine | Interaction with the ED medicine | What it means for you |
|---|---|---|
| Amiodarone (for irregular heartbeat) | Moderate CYP3A4 inhibitor; may increase sildenafil, tadalafil or vardenafil levels. Both amiodarone and PDE5 inhibitors can prolong QT interval. | Your prescriber will assess your heart health carefully. You may be started on a lower dose of the ED medicine. Report any dizziness, fainting or irregular heartbeat. |
| Other antiarrhythmics (e.g. sotalol, dronedarone) | May also affect heart rhythm or prolong QT interval. Some may interact with liver enzymes. | Full medication review needed. Prescriber will check if your heart is stable and whether sexual activity is safe for you. |
| Beta-blockers (e.g. bisoprolol, atenolol) | Can lower blood pressure; may add to the blood-pressure-lowering effect of PDE5 inhibitors. | Usually manageable. Declare all blood pressure medicines in your consultation. Start with the lowest ED dose and monitor for dizziness. |
| Calcium channel blockers (e.g. amlodipine, diltiazem) | Can lower blood pressure. Diltiazem and verapamil are also moderate CYP3A4 inhibitors. | Prescriber will review your blood pressure control and may adjust the ED dose. Report dizziness or light-headedness. |
What you need to know
- Sildenafil and amiodarone can be taken together, but only after careful prescriber assessment of your heart health.
- Amiodarone may increase sildenafil levels, so a lower starting dose is often recommended.
- Both medicines can affect heart rhythm; report dizziness, fainting or irregular heartbeat promptly.
- All ED treatments require a full medication review if you take amiodarone or other heart medicines.
- Never take sildenafil with nitrates, nicorandil, riociguat or poppers, as this can cause dangerous low blood pressure.
How does the interaction between sildenafil and amiodarone work?
The interaction between sildenafil and amiodarone happens in two main ways. First, amiodarone inhibits an enzyme in the liver called CYP3A4, which is responsible for breaking down sildenafil. When this enzyme is partially blocked, sildenafil levels in your blood can rise slightly, which may increase both the beneficial effects and the risk of side effects such as headache, flushing or dizziness. This is why a lower starting dose of sildenafil is often recommended if you take amiodarone.
Second, both amiodarone and sildenafil can affect the heart's electrical activity. Amiodarone is known to prolong the QT interval (a measurement on an ECG that reflects how long it takes for the heart to recharge between beats), and sildenafil can very rarely do the same. If the QT interval becomes too long, there's a small risk of a dangerous irregular heartbeat called torsades de pointes. This risk is higher if you take other medicines that also prolong the QT interval, have low potassium or magnesium levels, or have certain inherited heart conditions. Your prescriber will weigh up these factors carefully before prescribing.
What are the warning signs to watch for?
If you're prescribed sildenafil while taking amiodarone, it's important to be aware of warning signs that might suggest a problem. Contact your GP or call 111 if you experience new or worsening dizziness, fainting, a very fast or irregular heartbeat, or chest discomfort. These symptoms could indicate that the combination is affecting your heart rhythm or blood pressure. You should also watch for common sildenafil side effects such as severe headache, visual disturbances, or prolonged erection (lasting more than four hours), which requires urgent medical attention.
If you develop sudden chest pain during or after sexual activity, stop and seek urgent help, as this could be a sign of a heart problem. Sudden loss of vision in one or both eyes, or sudden decrease or loss of hearing (sometimes with ringing in the ears or dizziness) are rare but serious side effects of PDE5 inhibitors and require immediate medical attention. Always follow the dose prescribed and never take more than one dose of sildenafil in 24 hours.
When to seek urgent help
Call 999 or go to A&E immediately if you experience an erection lasting more than four hours (priapism), sudden loss of vision, severe chest pain, or if you faint or have a seizure. These are medical emergencies. For less urgent concerns, such as bothersome side effects or questions about your treatment, contact your GP, pharmacist or the prescribing service for advice. Never stop taking amiodarone or any other prescribed heart medicine without speaking to your doctor first, as this could be dangerous.
When to get urgent help
- Erection lasting more than 4 hours (priapism) – call 999 or go to A&E immediately
- Sudden loss of vision in one or both eyes – seek urgent medical attention
- Severe chest pain during or after sex – call 999 immediately
- Fainting, seizure or severe irregular heartbeat – call 999 or go to A&E
- Sudden loss of hearing or severe dizziness with ringing in the ears – seek same-day medical advice
Call 999 or go to A&E in an emergency, or contact NHS 111 for urgent advice.
Practical advice for taking sildenafil with amiodarone
If your prescriber decides that sildenafil is suitable for you, they will likely start you on the lowest effective dose (often 25mg) and may advise you to take it only occasionally at first to see how you respond. Take sildenafil about one hour before you plan to have sex, on an empty stomach or after a light meal, as a heavy or high-fat meal can delay its effects. Avoid drinking large amounts of alcohol, as this can make erectile dysfunction worse and increase the risk of dizziness or low blood pressure when combined with sildenafil.
Keep all your follow-up appointments with your GP or cardiologist, and let them know you're taking sildenafil. If you notice any new symptoms or side effects, report them promptly. Do not take sildenafil more often than prescribed, and never combine it with other ED medicines or with nitrates, nicorandil, riociguat or recreational drugs called 'poppers'. If sildenafil doesn't work well for you or causes troublesome side effects, speak to your prescriber about alternative ED treatments such as tadalafil, which has a different duration of action, or non-tablet options.
Alternative ED treatments if sildenafil isn't suitable
If your prescriber decides that sildenafil is not the best choice for you while taking amiodarone, there are other licensed ED treatments available in the UK. Tadalafil (Cialis) is another PDE5 inhibitor that works in a similar way but lasts much longer (up to 36 hours) and can also be taken as a low daily dose. Vardenafil (Levitra) and avanafil (Spedra) are also PDE5 inhibitors with slightly different profiles. All of these medicines require the same careful assessment of your heart health and drug interactions. Your prescriber will discuss which option is most suitable for your individual circumstances, and all treatments are available through Cured Pharmacy's online consultation service, reviewed by UK-registered prescribers.
Always declare amiodarone in your consultation
Be completely honest about all your medicines, including amiodarone, when filling in your online consultation. This allows the prescriber to assess the interaction safely and choose the right ED treatment and dose for you.

Your Questions Answered
Is it safe to take sildenafil if I'm on amiodarone?
Sildenafil can be prescribed with amiodarone, but only after a thorough medical assessment by a qualified prescriber who will review your heart health, amiodarone dose, and other medicines to ensure it's safe for you.
Will amiodarone make sildenafil less effective?
No, amiodarone may actually increase sildenafil levels slightly because it slows down how quickly your liver breaks down sildenafil. This is why a lower starting dose is often recommended.
Can I take Viagra if I have atrial fibrillation?
Having atrial fibrillation does not automatically rule out Viagra (sildenafil), but your prescriber will need to check that your heart condition is stable, that sexual activity is safe for you, and that your medicines (including amiodarone) don't create unsafe interactions.
What dose of sildenafil should I start with if I take amiodarone?
Your prescriber will decide the safest starting dose based on your individual circumstances, but it's often 25mg (the lowest dose) to minimise the risk of side effects while checking how you respond.
Are there any ED treatments that are safer with amiodarone?
All PDE5 inhibitors (sildenafil, tadalafil, vardenafil, avanafil) require the same careful assessment if you take amiodarone. Your prescriber will recommend the most suitable option based on your heart health, other medicines, and personal preferences.
Can I take tadalafil instead of sildenafil if I'm on amiodarone?
Tadalafil is another PDE5 inhibitor that may be suitable, but it also interacts with amiodarone in similar ways. Your prescriber will assess whether tadalafil, sildenafil or another ED treatment is best for you.
What should I do if I feel dizzy after taking sildenafil with amiodarone?
Sit or lie down until the dizziness passes, and avoid driving or operating machinery. If dizziness is severe, persistent, or accompanied by fainting or chest pain, seek medical advice promptly.
Do I need to tell my cardiologist I'm taking sildenafil?
Yes, it's important to keep your cardiologist informed about all medicines you take, including ED treatments, so they can monitor your heart health and adjust your treatment plan if needed.
