Not sure which one? Start a free consultation →Tadalafil and Pravastatin: Understanding the Interaction
Published: 24 October 2026
Many men taking pravastatin for high cholesterol wonder whether tadalafil and pravastatin can be taken together safely. The good news is that no major interaction is commonly reported between these two medicines, and they are often prescribed together. However, every person's situation is different, so your prescriber will review all your current medicines, health conditions, and blood pressure during the consultation to ensure tadalafil is suitable for you.
Keep taking your statin
Never stop or change your pravastatin dose without speaking to your GP. It is an important medicine for your heart health, and improving your cardiovascular health may also help improve erectile function over time.
What is tadalafil and what does it treat?
Tadalafil is a prescription medicine used to treat erectile dysfunction (ED) in men [4]. It belongs to a group of medicines called PDE5 inhibitors, which work by relaxing blood vessels in the penis, allowing more blood to flow in when you are sexually aroused. Tadalafil does not cause an erection on its own—sexual stimulation is still needed for it to work [4].
Tadalafil is available as the branded medicine Cialis and as generic versions. It can be taken in two ways: on-demand (10mg or 20mg taken at least 30 minutes before sexual activity, with effects lasting up to 36 hours) or daily (2.5mg or 5mg taken once a day at the same time, allowing for more spontaneity) [4]. The long duration of action is why tadalafil is sometimes called the 'weekend pill'.
At Cured Pharmacy, tadalafil is available after an online consultation reviewed by a UK-registered prescriber. The prescriber will check your medical history, current medicines, and blood pressure to ensure tadalafil is safe and appropriate for you.
What is pravastatin and why is it prescribed?
Pravastatin is a statin medicine prescribed to lower cholesterol levels in the blood. High cholesterol can lead to fatty deposits building up in blood vessels, increasing the risk of heart attack, stroke, and other cardiovascular problems. Pravastatin works by blocking an enzyme in the liver that produces cholesterol, helping to reduce the amount of 'bad' LDL cholesterol and triglycerides while raising 'good' HDL cholesterol.
Statins like pravastatin are often prescribed alongside lifestyle changes such as a healthy diet, regular exercise, and stopping smoking. They are particularly important for people with diabetes, a history of heart disease, or other risk factors for cardiovascular disease. Pravastatin is usually taken once daily, often in the evening, and is generally well tolerated.
Interestingly, high cholesterol and erectile dysfunction often occur together because both can be caused by damage to blood vessels. Atherosclerosis (narrowing of the arteries) affects blood flow throughout the body, including to the penis. This is why treating high cholesterol is an important part of overall cardiovascular health, which in turn may help improve erectile function over time.
When to get urgent help
- Chest pain during or after sexual activity—call 999 and do not take nitrates if you have used tadalafil
- Erection lasting more than 4 hours (priapism)—go to A&E immediately to prevent permanent damage
- Sudden loss of vision in one or both eyes—stop tadalafil and seek urgent medical help

Call 999 or go to A&E in an emergency, or contact NHS 111 for urgent advice.
Do tadalafil and pravastatin interact with each other?
No major interaction is commonly reported between tadalafil and pravastatin, and these medicines are frequently prescribed together. Pravastatin is not a potent inhibitor of the liver enzyme (CYP3A4) that breaks down tadalafil, so it does not significantly raise tadalafil levels in the blood. This is different from some other medicines that do affect this enzyme and require dose adjustments.
However, it is important to remember that both medicines can have an effect on blood pressure. Tadalafil can cause a small, temporary drop in blood pressure, and while pravastatin itself does not usually lower blood pressure, the overall cardiovascular health of someone taking a statin needs to be considered. Your prescriber will check your blood pressure and review your full medical history to ensure the combination is safe for you.
During your online consultation at Cured Pharmacy, you will be asked to list all your current medicines, including pravastatin and any other cholesterol or blood pressure tablets. This allows the prescriber to check for any interactions and ensure tadalafil is prescribed at the right dose for your individual circumstances. Never stop or change your pravastatin without speaking to your GP, as this is an important medicine for your heart health.
How the prescriber checks safety
When you request tadalafil, the prescriber will ask about your current medicines, including pravastatin, and any other health conditions such as heart disease, high or low blood pressure, diabetes, or kidney problems. They will also check whether you take any medicines that are known to interact with tadalafil, such as nitrates (used for angina), alpha-blockers (used for prostate or blood pressure problems), or potent CYP3A4 inhibitors [4].
The prescriber will review your blood pressure and cardiovascular health to ensure that sexual activity is safe for you. If you have recently had a heart attack or stroke, have unstable angina, or have severe heart failure, tadalafil may not be suitable [4]. The consultation is designed to identify any risks and ensure you receive the right treatment safely.
| Medicine | Interaction with tadalafil | What it means for you |
|---|---|---|
| Pravastatin | No major interaction commonly reported | Usually safe to take together; prescriber checks your blood pressure and overall health during consultation |
| Atorvastatin | No major interaction commonly reported | Can usually be taken with tadalafil; prescriber reviews your full medical history and current medicines |
| Simvastatin | No major interaction commonly reported | Generally safe to combine; prescriber checks for any other interacting medicines and cardiovascular health |
| Rosuvastatin | No major interaction commonly reported | Typically prescribed together without issue; full consultation ensures safety for your individual circumstances |
| Fibrates (e.g. fenofibrate, bezafibrate) | No major interaction commonly reported | Can usually be taken with tadalafil; prescriber reviews all cholesterol-lowering medicines and checks blood pressure |
Are there any warning signs to watch for?
While tadalafil and pravastatin are generally safe to take together, it is important to be aware of warning signs that need urgent medical attention. If you experience chest pain during or after sexual activity, stop and seek immediate medical help, as this could be a sign of a heart problem. Do not take nitrate medicines (such as GTN spray) to relieve chest pain if you have taken tadalafil, as this combination can cause a dangerous drop in blood pressure [4].
Other urgent warning signs include an erection that lasts more than four hours (priapism), which requires immediate treatment to prevent permanent damage to the penis. Sudden loss of vision in one or both eyes, or sudden decrease or loss of hearing (sometimes with ringing in the ears or dizziness), are rare but serious side effects that require you to stop taking tadalafil and seek medical help straight away [4].
Common side effects of tadalafil include headache, indigestion, back pain, muscle aches, flushing, and a blocked or runny nose [4]. These are usually mild and temporary. If you experience dizziness or feel faint, especially when standing up, sit or lie down until it passes. If dizziness persists or you feel unwell, contact your GP or pharmacist for advice.

Practical advice for taking tadalafil and pravastatin together
If you are prescribed tadalafil while taking pravastatin, continue taking your pravastatin exactly as your GP has directed. Do not stop or change the dose of your statin without medical advice, as this is important for your heart health. Take tadalafil as prescribed—either on-demand (at least 30 minutes before sexual activity) or once daily at the same time each day [4].
Avoid drinking large amounts of alcohol when taking tadalafil, as this can increase the risk of dizziness, headaches, and low blood pressure, and may make it harder to get an erection. Grapefruit juice can raise the level of tadalafil in your blood, so it is best to avoid it or limit your intake. If you are taking tadalafil daily, try to take it at a time that is easy to remember and fits with your routine.
Keep all your medicines in their original packaging, store them safely away from children, and check the expiry dates. If you are prescribed a new medicine by your GP or another doctor, always tell them you are taking tadalafil and pravastatin. If you have any questions or concerns about your medicines, contact the Cured Pharmacy team or speak to your GP.
Lifestyle tips to support your treatment
Erectile dysfunction and high cholesterol often share the same underlying causes, such as poor diet, lack of exercise, smoking, and being overweight [1]. Making healthy lifestyle changes can improve both conditions. Aim for at least 150 minutes of moderate exercise each week, eat a balanced diet rich in fruit, vegetables, whole grains, and healthy fats, and try to maintain a healthy weight.
If you smoke, stopping is one of the best things you can do for your heart and sexual health. Smoking damages blood vessels and reduces blood flow throughout the body, including to the penis. Your GP can refer you to a free NHS stop-smoking service for support. Managing stress and getting enough sleep are also important for overall wellbeing and can help improve erectile function.
Other medicines and health conditions to consider
While tadalafil and pravastatin can usually be taken together, there are some medicines that must never be taken with tadalafil. These include nitrates (such as GTN spray, isosorbide mononitrate, or isosorbide dinitrate), nicorandil, riociguat, and recreational drugs called 'poppers' (amyl or alkyl nitrites). Combining tadalafil with any of these can cause a dangerous drop in blood pressure [4]. Do not take tadalafil if you are using another PDE5 inhibitor such as sildenafil or vardenafil.
If you take alpha-blockers (such as doxazosin, tamsulosin, or alfuzosin) for prostate problems or high blood pressure, tell your prescriber, as these can interact with tadalafil and increase the risk of dizziness or fainting. You should be stable on your alpha-blocker before starting tadalafil, and the prescriber may start you on a lower dose [4]. Similarly, if you take other blood pressure medicines, the prescriber will check that the combination is safe.
Some medicines can raise the level of tadalafil in your blood by affecting how it is broken down in the liver. These include potent CYP3A4 inhibitors such as ritonavir, cobicistat (found in some HIV medicines), ketoconazole, itraconazole, and clarithromycin. If you take any of these, your tadalafil dose may need to be reduced [4]. Always declare all your current medicines during the consultation, including over-the-counter medicines, herbal remedies, and supplements.
When to see your GP about erectile dysfunction
If you are experiencing erectile dysfunction for the first time, it is important to see your GP for a check-up. ED can be an early warning sign of underlying health problems such as heart disease, diabetes, high blood pressure, or high cholesterol [1]. Your GP can arrange blood tests and other checks to identify any underlying causes and ensure you receive the right treatment.
Your GP can also review your current medicines, as some can contribute to ED, and discuss lifestyle changes that may help. If ED is affecting your mental health or relationship, your GP can offer support or refer you to a specialist if needed. Treating the underlying cause, alongside ED medication if appropriate, gives you the best chance of improvement.
