Sildenafil and Poppers: A Dangerous Combination

Published: 7 November 2026

Sildenafil and poppers must never be taken together. Both medicines lower blood pressure, and when combined they can cause a sudden, severe drop that may lead to fainting, stroke, heart attack, or even death. Poppers—also known as amyl nitrite, butyl nitrite, or alkyl nitrites—are recreational drugs sometimes used to enhance sexual experience, but they are absolutely contraindicated with all erectile dysfunction (ED) treatments that belong to the PDE5 inhibitor family, including sildenafil (Viagra), tadalafil (Cialis), vardenafil (Levitra), and avanafil (Spedra). This page explains what each substance does, how the interaction works, and the vital safety checks that protect you.

What Is Sildenafil and How Does It Work?

Sildenafil is a prescription medicine used to treat erectile dysfunction in men. It is the active ingredient in Viagra and is also available as a generic medicine at lower cost. Sildenafil belongs to a group of medicines called phosphodiesterase type 5 (PDE5) inhibitors [3].

When you are sexually aroused, your body releases a chemical messenger called nitric oxide, which helps relax the smooth muscle in the blood vessels of the penis. This allows more blood to flow in, producing an erection. An enzyme called PDE5 normally breaks down the messenger that keeps those blood vessels relaxed. Sildenafil blocks PDE5, allowing the blood vessels to stay open longer and making it easier to get and keep an erection when you are sexually stimulated [3].

Sildenafil is usually taken about one hour before sexual activity. It comes in 25 mg, 50 mg, and 100 mg tablets, and the effect can last for around four to six hours. You should not take it more than once a day. Sildenafil does not cause an erection on its own—you still need to be sexually aroused for it to work [3].

Because sildenafil works by relaxing blood vessels, it also has a mild blood-pressure-lowering effect throughout the body. This is usually well tolerated, but it becomes dangerous when sildenafil is combined with other medicines or substances that also lower blood pressure, especially nitrates and nitrites such as poppers [3].

What Are Poppers and Why Are They Used?

Poppers are a group of recreational drugs that contain alkyl nitrites, most commonly amyl nitrite, butyl nitrite, or isobutyl nitrite. They are usually sold in small bottles as liquids that give off a vapour, which is inhaled. The name 'poppers' comes from the popping sound made when the glass capsules were originally broken open [1].

When inhaled, poppers cause the smooth muscle around blood vessels to relax very quickly, leading to a sudden drop in blood pressure. This produces a brief 'rush' or feeling of light-headedness, warmth, and relaxation that lasts for a minute or two. Some people use poppers during sex because they believe the relaxation of smooth muscle enhances sexual pleasure [1].

Although poppers are sometimes sold in sex shops, online, or in clubs, they are not safe recreational drugs. They can cause serious side effects including headaches, dizziness, nausea, and in rare cases damage to the eyes or blood cells. More importantly, poppers are chemically related to the nitrate medicines used to treat angina (chest pain caused by heart disease), and they have the same dangerous interaction with sildenafil and other PDE5 inhibitors [3].

It is illegal to sell poppers for human consumption in the UK under the Psychoactive Substances Act 2016, although enforcement varies. Regardless of their legal status, poppers are medically contraindicated with all ED treatments, and you must never use them together.

Why Sildenafil and Poppers Must Never Be Taken Together

The combination of sildenafil and poppers is extremely dangerous because both substances work by relaxing blood vessels and lowering blood pressure. When taken together, their effects add up and can cause a sudden, severe drop in blood pressure that your body cannot compensate for quickly enough [3].

Sildenafil enhances the effect of nitric oxide by blocking the enzyme (PDE5) that would normally limit how long blood vessels stay relaxed. Poppers release nitric oxide directly into your bloodstream when inhaled. The result is that blood vessels throughout your body—including those supplying your brain, heart, and other vital organs—relax too much, and your blood pressure can plummet within minutes [3].

This dangerous drop in blood pressure can cause fainting, dizziness, chest pain, heart attack, stroke, or sudden death. Even a small amount of poppers taken hours after sildenafil can trigger this reaction, because sildenafil stays in your system for several hours. The risk is not theoretical—there have been documented cases of serious harm and death from this combination [3].

For this reason, sildenafil (and all other PDE5 inhibitors) are absolutely contraindicated with poppers and all other nitrites and nitrates. If you use poppers recreationally, you must not take sildenafil or any other ED medicine. If you are prescribed sildenafil, you must avoid poppers completely. There is no safe way to combine these substances [3].

How Long Should You Wait?

Because sildenafil can remain active in your body for several hours, and because the risk of a dangerous blood pressure drop is so serious, there is no safe minimum waiting time between taking sildenafil and using poppers. The only safe approach is to avoid poppers entirely if you are using any PDE5 inhibitor for erectile dysfunction [3].

Sildenafil is usually cleared from your body within 24 hours, but the medicine's effect on your blood vessels can last longer in some people, especially if you have kidney or liver problems, or if you are taking other medicines that slow down how your body processes sildenafil. Even waiting a day or two does not guarantee safety, because individual responses vary [3].

If you have accidentally used poppers while taking sildenafil, or if you have taken sildenafil and then used poppers, seek urgent medical help immediately—even if you feel well at first. Do not wait for symptoms to develop. Call 999 or go to your nearest A&E department and tell the healthcare team exactly what you have taken and when.

Substance Interaction with Sildenafil What It Means for You
Poppers (amyl nitrite, butyl nitrite, alkyl nitrites) Absolute contraindication—causes dangerous drop in blood pressure Do not take together. Never use poppers if you are taking sildenafil or any ED medicine. Seek urgent help (999) if combined.
Nitrate medicines (GTN spray, isosorbide mononitrate, isosorbide dinitrate) Absolute contraindication—causes dangerous drop in blood pressure Do not take together. Tell your prescriber if you use nitrates for angina. Sildenafil will not be prescribed.
Nicorandil (angina medicine) Absolute contraindication—causes dangerous drop in blood pressure Do not take together. Tell your prescriber if you take nicorandil. Sildenafil will not be prescribed.
Alpha-blockers (doxazosin, tamsulosin, alfuzosin) Risk of low blood pressure and dizziness May be used together with caution. You must be stable on the alpha-blocker first, and your prescriber will start you on the lowest dose of sildenafil.
Other blood pressure medicines (e.g. amlodipine, ramipril, bisoprolol) Additive blood-pressure-lowering effect; usually manageable Declare all blood pressure medicines during your consultation. Your prescriber will check it is safe and may adjust your dose.

What you need to know

  • Sildenafil and poppers must never be taken together—the combination can cause a life-threatening drop in blood pressure.
  • Poppers are alkyl nitrites that relax blood vessels; sildenafil enhances this effect, leading to dangerous cardiovascular collapse.
  • There is no safe waiting time between sildenafil and poppers—avoid poppers completely if you use any ED medicine.
  • If you have taken both, call 999 immediately, even if you feel well. Tell healthcare staff exactly what you have taken.
  • Always declare recreational drug use during your ED consultation—it is confidential and essential for your safety.

What Your Prescriber Checks Before Prescribing Sildenafil

Before sildenafil or any other ED medicine can be prescribed, a UK-registered prescriber (doctor, pharmacist prescriber, or nurse prescriber) must carry out a clinical assessment to make sure the treatment is safe for you. This assessment includes questions about your current and past medical conditions, all medicines you are taking (including over-the-counter, herbal, and recreational substances), and your cardiovascular health [2].

The prescriber will specifically ask whether you use nitrate medicines for angina (such as glyceryl trinitrate spray, isosorbide mononitrate, or isosorbide dinitrate), nicorandil (another angina medicine), riociguat (for pulmonary hypertension), or recreational poppers. If you use any of these, sildenafil will not be prescribed because the combination is contraindicated [3].

You will also be asked about alpha-blocker medicines (used for high blood pressure or prostate problems, such as doxazosin or tamsulosin), other blood-pressure medicines, and any drugs that affect how your liver processes medicines (such as some HIV treatments, antibiotics, or antifungals). These may not rule out sildenafil completely, but they may mean you need a lower starting dose or extra monitoring [3].

The prescriber will check whether you have had a recent heart attack or stroke, unstable angina, severe heart failure, very low blood pressure, or previous sudden vision loss (a rare condition called non-arteritic anterior ischaemic optic neuropathy, or NAION). Any of these conditions may mean sildenafil is not suitable, because sexual activity itself or the blood-pressure effects of the medicine could be risky [2][3].

It is essential that you answer all questions honestly and completely, including questions about recreational drug use. The consultation is confidential, and the prescriber is not there to judge you—they need accurate information to keep you safe. If you use poppers or any other recreational substance, you must declare it, even if you find it embarrassing. Withholding this information could put your life at risk.

Warning Signs and When to Seek Urgent Help

If you have taken sildenafil and poppers together—whether by accident or because you were unaware of the risk—you may experience symptoms of dangerously low blood pressure. These can include severe dizziness or light-headedness, fainting or feeling like you are about to faint, a rapid or pounding heartbeat, blurred vision, confusion, nausea, chest pain, or difficulty breathing [3].

These symptoms can develop very quickly, sometimes within minutes of inhaling poppers. Even if you feel only mildly unwell at first, the situation can deteriorate rapidly. Do not wait to see if you feel better. Call 999 immediately, tell the call handler that you have taken sildenafil and poppers, and follow their instructions. If you are with someone who has taken this combination and becomes unwell, call 999 on their behalf and stay with them until help arrives [3].

There are also other warning signs related to sildenafil itself that require urgent medical attention, even if you have not used poppers. If you develop an erection that lasts longer than four hours (priapism), you must go to A&E straight away, as this can cause permanent damage to the penis. Sudden loss of vision in one or both eyes, or sudden decrease or loss of hearing (sometimes with ringing in the ears or dizziness), are rare but serious side effects that need immediate assessment [3].

If you experience chest pain during or after sex, stop the activity and seek medical help. Do not take any nitrate medicine (such as GTN spray) to relieve the chest pain if you have taken sildenafil within the last 24 hours, as this can cause the same dangerous drop in blood pressure as poppers. Tell the paramedics or A&E staff that you have taken sildenafil so they can treat you safely [3].

What to Tell Healthcare Professionals

If you need emergency treatment and you have taken sildenafil, poppers, or both, it is vital that you tell the healthcare team exactly what you have taken, how much, and when. This includes the dose of sildenafil, the time you took it, and whether you have used any other medicines or substances, including alcohol or recreational drugs [3].

Healthcare professionals are not there to judge you. They need this information to treat you safely and effectively. For example, if you are having chest pain and the team does not know you have taken sildenafil, they might give you a nitrate medicine, which could cause a life-threatening drop in blood pressure. Being honest and clear about what you have taken could save your life [3].

When to get urgent help

  • Severe dizziness, fainting, or feeling like you are about to faint after taking sildenafil and poppers—call 999 immediately.
  • Chest pain during or after sex—stop activity, call 999, and tell them you have taken sildenafil (do not take GTN spray or any nitrate).
  • An erection lasting longer than 4 hours (priapism)—go to A&E straight away to prevent permanent damage.
  • Sudden loss of vision in one or both eyes, or sudden hearing loss—seek urgent medical help the same day.
  • Rapid or irregular heartbeat, difficulty breathing, or confusion after combining sildenafil with any substance—call 999 and tell them what you have taken.

Call 999 or go to A&E in an emergency, or contact NHS 111 for urgent advice.

Practical Advice: Staying Safe with ED Treatment

If you are prescribed sildenafil or another PDE5 inhibitor for erectile dysfunction, the most important safety rule is simple: do not use poppers or any other nitrite or nitrate substance. This includes amyl nitrite, butyl nitrite, and any product sold as a 'room odoriser', 'leather cleaner', or similar euphemism that is actually intended for inhalation [3].

If you have used poppers in the past and are now considering ED treatment, you must stop using poppers completely before starting sildenafil. There is no safe way to use both. If you find it difficult to stop using poppers, talk to your GP or a sexual health service—they can offer support and advice without judgement [1].

When you have your online consultation for ED treatment, read each question carefully and answer truthfully. If the form asks about nitrate medicines or recreational drugs, do not skip the question or assume it does not apply to you. Poppers are nitrites, and they are just as dangerous with sildenafil as prescription nitrate medicines for angina [3].

Keep your sildenafil in its original packaging with the patient information leaflet, and read the leaflet before you take your first dose. It will list all the medicines and substances you must not take with sildenafil. If you are ever unsure whether something is safe to use, ask a pharmacist or your prescriber before taking it [3].

If you are in a situation where poppers are being used by others (for example, at a party or in a club), be aware that inhaling the vapour even passively could be risky if you have taken sildenafil. It is best to avoid environments where poppers are being used if you are taking any ED medicine. Your health and safety are more important than any social pressure [3].

Other Medicines and Substances to Avoid

In addition to poppers, sildenafil must not be taken with any nitrate medicine used for angina (such as glyceryl trinitrate spray, isosorbide mononitrate, or isosorbide dinitrate), nicorandil (another heart medicine), or riociguat (used for pulmonary hypertension). You must also not take sildenafil at the same time as another PDE5 inhibitor (such as tadalafil, vardenafil, or avanafil), as this increases the risk of side effects without improving the effect [3].

Grapefruit juice can increase the levels of sildenafil in your blood and raise the risk of side effects, so it is best to avoid it. Heavy alcohol can make erectile dysfunction worse, increase the risk of side effects such as dizziness and low blood pressure, and make it harder for sildenafil to work effectively. If you choose to drink alcohol, keep it to a moderate amount [3].

The Most Important Safety Rule

If you are prescribed sildenafil or any ED medicine, you must stop using poppers completely. There is no safe way to combine them, and the risk of a life-threatening drop in blood pressure is real. Always tell your prescriber about any recreational substance use—it is confidential and could save your life.

Common Questions

Can I take sildenafil if I have used poppers in the past?

Yes, as long as you stop using poppers completely before starting sildenafil and do not use them again while taking any ED medicine. There is no safe way to combine sildenafil and poppers.

How long after taking sildenafil can I safely use poppers?

You must not use poppers at all if you are taking sildenafil or any other PDE5 inhibitor. There is no safe waiting period—the only safe approach is to avoid poppers entirely.

What should I do if I accidentally took sildenafil and poppers together?

Call 999 immediately or go to your nearest A&E department, even if you feel well. Tell the healthcare team exactly what you have taken and when. Do not wait for symptoms to develop.

Are poppers the same as nitrate medicines for angina?

Poppers contain alkyl nitrites, which are chemically related to nitrate medicines (which contain organic nitrates). Both have the same dangerous interaction with sildenafil and must never be taken together.

Will my prescriber know if I use poppers?

Your prescriber will ask you directly during the consultation whether you use poppers or any recreational drugs. You must answer honestly—the consultation is confidential, and withholding this information could put your life at risk.

Can I take a lower dose of sildenafil if I want to use poppers?

No. The interaction between sildenafil and poppers is not dose-dependent—even a low dose of sildenafil with poppers can cause a dangerous drop in blood pressure. Do not take them together at any dose.

Are there any ED treatments that are safe to use with poppers?

No. All PDE5 inhibitors (sildenafil, tadalafil, vardenafil, and avanafil) are contraindicated with poppers. Other prescription ED treatments such as alprostadil may have different risks, but you must discuss recreational drug use with your prescriber before starting any ED medicine.

What are the symptoms of dangerously low blood pressure from this combination?

Symptoms include severe dizziness, fainting, rapid heartbeat, blurred vision, confusion, nausea, chest pain, or difficulty breathing. If you experience any of these after taking sildenafil and poppers, call 999 immediately.
